A Cardiff-based printing manufacturer has been fined £400,000 after an engineer suffered fatal injuries from a workplace accident whilst he was repairing an industrial overhead door.
The accident occurred on 5 September 2022 at the company’s manufacturing site, where 59-year-old maintenance worker Anthony Webb was tasked with repairing an electrically operated sectional overhead door. During the repair, he attempted to re-tension the door’s springs using an industrial wrench, which slipped unexpectedly, causing the spring mechanism to unwind with significant force and eject the wrench. This struck Mr Webb, who sustained catastrophic injuries and sadly passed away the following day.
An investigation by the Health and Safety Executive (HSE) found that the company had allowed Mr Webb to carry out repairs on multiple previous occasions, despite him not having received the appropriate training for this specialist work. They also identified several other failings leading to this fatal workplace accident, including not carrying out a suitable risk assessment, not implementing a safe system of work, and not providing the appropriate equipment needed to complete the repair safely.
